Doug Ingraham retyped the source from the write-up.

He commented out the PAUSE and appended the two source halves.  Possibly
there should be -pa1 and -pa2, rather than just a single -pa, but I have
left it this way for now.

I've removed some comments Doug inserted where he described ambiguities
and minor problems in the write-up.  This hopefully makes the -pa a
good approximation to the original DEC source.
